Members who wish to submit a charity for consideration must complete a Charity Nomination Form (below). Alternatively, the PDF NOMINATION FORM  may be printed and scanned/emailed or mailed prior to the meeting (instructions are provided on the form). 

 

The form must be submitted at least 7 days before the meeting date in order to be eligible for consideration at the upcoming meeting.  Only 501(c)(3) charities that have been in existence for a minimum of two years will be considered. Nominated charities should benefit our local communities. Please see the main CHARITIES page for additional information about the charity selection process.
